Transcode Fails after Upgrade on Windows Host for Plex Media Server

Server: Plex 1.0.0.2261
Host OS: Windows 2012 R2
Tested with Web: Firefox 47.01
Tested with Roku: Latest (not sure version)

MKV files, and other file types do not play after upgrade.

The issue I'm having started after my update three versions ago, there were three quick upgrades (9.16, 9.17, and then 1.0...). All was working fine prior to that. I have seen this error reported with the same exact symptoms but the solutions were either issues with space (I have 160GB free on the OS partition, moved the temp transcode to my data partition with 2.4TB free) or they were on Linux (forums.plex.tv/discussion/comment/1204390/#Comment_1204390) and the Linux solution will not work as this is on a Windows box. I scanned my entire system for noexec using SearchMyFiles and the only file found related to Plex was the actual Plex executable.

When running in web, it spins and does not play.
When running on Roku, report 13%, 33%, and restarts. Does this three times and then fails with error the same as in this thread https://forums.plex.tv/discussion/223941/half-my-collection-of-mpg-music-videos-stopped-playing-on-all-devices-with-upgrade-to-1-0-0#latest, but they are on a NAS: "Video Unavailable - We're unable to play this video, make sure the server is running and has access to this video"
On the ROKU, the progress goes to 13% then fails with the error message, "Video Unavailable - We're unable to play this video, make sure the server is running and has access to this video"

Test file is Preacher S01E05. Tested in Firefox on the Plex server.

I have attached logs after stopping PMS and then starting again, playing file, waiting five minutes, stop file and zipped logs.
