My server has become unstable, often "crashes", issues, buffering and more.

Hi Plex Community,

I really hope someone can help me, as i'm slowly losing my mind.
I have a Synology NAS station for storage, and a dedicated PC i built solely for Plex. It's running with an i7 930, 64GB ram and a discreet video card, it's running Windows 7.

It's been working great for months, but in the last couple of weeks, there have been numerous issues, i just can't seem to figure out why.

The problems range from everything to:
-The server randomly disappears for a few seconds, before regaining connection
-Casting from Chromecast will occasionally "crash" the plex (Not a "true" crash, but i lose connection to server for a few seconds)
- "video conversion failed. the transcoder crashes or failed to start up" also happens from time to time, a couple of F5's will fix it
- Manually matching metadata will often fail, and "crash" the server.
- During auto refresh, the server becomes unavailable for 30 to 60 seconds
- "Remote play will suddenly fail, but will auto regain connection after about 10 seconds.
- "Direct connection available, attempting to relay connection"

As i said above, the server is dedicated to Plex, i have never installed anything on it besides Windows 7, PlexPy, PMS, and Speedfan.
My internet connection is 120mbit/50mbit, and very stable, so i doubt that's the issue.
I have a Netgear Nighthawk router, with port 32400 open.
The issues happens both remotely and locally, seemingly no difference between the two regarding the issues.
There is no difference if i'm the only one streaming something, or if there is 5 streamers at the same time.
The server temps are below 60c at all times, so it's not overheating

As mentioned, i havn't changed anything, and it used to work just fine with 100% uptime, my content is hosted on a NAS, so seperate from the PMS. (Connected through the router with cat6 cables) I keep Plex and Plexpy fully updated. I've tried disabling Plexpy, but that didn't fix the problem.

I have attached a log file, gathered around 10 seconds after a "video conversion failed" bug, but i have saved multiple logs if needed.
The plex never actually fully crashes, so there is no crash logfiles, unfortunately.

I really, really hope someone have some tips/tricks or other things i can try, and would greatly appreciate any help.

Thank you very much in advance and please ask if you need any more info.
